Please read these Terms & Conditions before submitting the Patient Photo & Treatment Video Consent Form.

1. What you are consenting to

By submitting the consent form, you give Bourne Dental Group permission to take photographs and/or videos of you and your dental treatment. Depending on the options you tick, your consent allows us to show and use: your teeth and smile, your eyes, your whole face, your dental treatment and its progress, before-and-after images or videos, and your appearance in photographs or videos taken during or in connection with your treatment.

Your photographs and videos may identify you, particularly where your whole face is visible. We will only show the parts of you that you selected on the form.

2. How your photos and videos may be used

Marketing and promotion — our practice websites, social media, online and digital advertising, printed materials such as brochures and leaflets, and other promotional campaigns controlled by Bourne Dental Group.

Dental education — dental education and professional training, educational presentations and conferences, and professional learning materials.

We will use your content only for the purposes described here and will not knowingly use it for an unrelated purpose without taking appropriate steps where required.

3. Your consent is voluntary

Giving consent is completely voluntary. You do not have to agree in order to receive dental treatment, and refusing will not affect your treatment, your care, your relationship with the practice or your access to dental services.

4. Your personal information

We may process the information you provide on the form — your name, email address, telephone number and practice — together with the photographs and videos themselves, any information visible in them, treatment-related information they contain, and your consent record. Dental photographs and videos may reveal information about your health or dental treatment.

We process your information in accordance with UK data-protection law, including the UK GDPR and the Data Protection Act 2018. Where required, we identify and document the lawful basis for processing and the condition for processing special-category health information.

5. Data protection and security

We take appropriate technical and organisational measures to protect your personal information, photographs and videos from unauthorised access, loss, alteration or misuse. Only people who need them for legitimate practice purposes can access them.

6. Online and social media use

Once photographs or videos are published online they may be viewed, downloaded, copied or shared by other people, and we cannot guarantee that third parties will not copy or redistribute them. If you withdraw your consent we will take reasonable steps to stop using the content on channels we control, but we cannot guarantee the removal of copies already made or shared by others.

7. Before-and-after content

Where photographs or videos show treatment results they may be used as before-and-after material. We take reasonable steps to make sure such material is genuine and does not give a misleading impression, and we follow UK advertising rules, including the CAP Code and ASA/CAP guidance on before-and-after photos.

8. Testimonials

If you separately give us a testimonial, statement or recorded comment, we may use it together with your approved photographs or videos. We will not change your words in a way that creates a misleading impression, and testimonials will reflect your genuine experience.

9. Withdrawing your consent

You can withdraw your consent at any time — it is as easy to withdraw as it was to give (ICO guidance). To withdraw, email [email protected], call 01980 847600, use our contact page or speak to your practice.

Withdrawal does not affect the lawfulness of any use before you withdrew. After we receive your request we will take reasonable steps to stop future use of your content under our control.

10. Consent records

We keep a record of your consent, including your name, the date and time you gave it, the version of the form and of these Terms & Conditions, the options you selected, how you gave consent, and any later withdrawal or change. This lets us show what you agreed to and manage your preferences.

11. Advertising and professional standards

We follow UK advertising and professional requirements when using patient photographs, videos and testimonials. Our marketing will not knowingly mislead patients about treatments, results, expected outcomes, effectiveness or patient experiences, and objective claims are supported by appropriate evidence. Another patient's photographs or testimonial do not guarantee that you will achieve the same result.

12. Changes to these terms

We may update these Terms & Conditions to reflect changes in law, regulation, professional guidance, advertising rules or our procedures. If a change materially affects how your consent is used, we will consider whether further information or consent is needed.

14. Your confirmation

By submitting the consent form, you confirm that you have read these Terms & Conditions; that you understand photographs and/or videos may show the parts of you that you selected and your dental treatment; that approved content may be used for marketing, promotion and dental education; that taking part is voluntary and does not affect your treatment; that you can withdraw at any time; and that content published online may be copied or shared by others.
